Description

A patient’s journey in cancer care, from their work up to their treatment and recovery, requires a systematic, integrated and coordinated approach to allow for the best outcomes and the best possible experience. Kaiser Permanente South Bay Medical Center’s Cancer Care Navigation Program creates a seamless process between Primary Care, Diagnostic Imaging, Pathology, Specialty Care and Surgical Specialties to fast track our patients in receiving the appropriate care from the time of cancer suspicion to diagnosis, treatment and ultimately to survivorship.
